Our Registration Center has all of the registration forms that you made need to register your child for our sports programs. These forms are in microsoft format for easy download. If you do not see a form you need please feel free to contact us and we will send you the correct form.

To register for our LSW programs you must register as an LSW Member. You can pay online or mail in a check prior to the start of the season.
